Personalised continence support for everyday comfort and independence
If you live in Deer Park or the surrounding western suburbs of Melbourne and need support with bladder or bowel health, A1 Care Group is here to help. Our continence nurses provide detailed assessments, personalised care plans and practical strategies that make daily life easier, safer and more comfortable.
We understand how sensitive continence issues can feel. Our team works with care, respect and privacy, offering clear guidance that fits your routine and helps you stay confident and independent.
A continence assessment is a nursing assessment that reviews your bladder and bowel health, daily patterns and any challenges you may be experiencing. It helps identify the right supports, products and routines to improve your comfort and wellbeing.
Your continence assessment includes:
A detailed review of your continence symptoms
A personalised continence assessment form and care plan
A clear, NDIS-ready continence report
Product recommendations such as pads, bedding protection and toileting aids
Practical bladder and bowel management strategies
Guidance for carers and support workers
Advice on toileting position, hydration, diet and mobility
Support for urinary incontinence, bowel incontinence, constipation or urgency
Your care plan is simple, realistic and tailored to your daily life.
How the Assessment Works
Your assessment is completed by a qualified Continence Nurse Consultant or Registered Nurse. We offer in-home appointments in Deer Park, and telehealth sessions for people who prefer remote support.
During your appointment, we:
Talk through your current continence challenges
Review the continence products you are using
Identify environmental, behavioural or physical factors affecting your continence
Offer practical toileting and hygiene advice
Suggest products suited to your needs and budget
Develop a personalised continence care plan
Prepare a detailed continence report for your NDIS plan
We take our time and make sure everything is explained in a way that feels simple and supportive.
